Text Neck: How to Protect Your Neck From Phone and Laptop Slouch

Looking down at a phone or laptop puts extra load on your neck, but this is usually muscle strain. A few changes in how you hold your phone and set up your screen, plus a short daily reset, usually ease it.

I am a spine surgeon at P.D. Hinduja Hospital, Mahim, Mumbai. Neck and upper back ache from phones and laptops is one of the commonest worries I hear, and most of it is fixable. This post covers what "text neck" really is, how to hold your phone, how to set up a laptop, and a short daily reset.

📱 What is text neck, and is it serious?

Text neck is a popular name for neck and shoulder aching after long spells of looking down at a screen. It isn't a medical diagnosis. In most people it is muscle strain. The muscles at the back of your neck and across your shoulders work harder to hold your head up. A review of studies found more activity in these muscles, and a more forward head position, during phone use [2].

⚖️ How much extra load does a bent neck carry?

In a neutral position your head weighs about 5 kg. A computer model estimated the load on the neck as the head tips forward [1]:

These are estimates from a model, not measurements in people. They do show why a long slump (when you are doom scrolling) tires your neck more than a quick glance. The same logic applies to a laptop on your lap or a low desk, where your head drifts forward and down for hours.

🤳 How should you hold your phone?

• 📱 Raise the phone to eye level. Bring it up to your face. Don't lower your face to it.
• 💪 Support your elbows. Rest them on a table, or tuck one forearm across your ribs and rest the other elbow on it.
• 👀 Look down with your eyes, not your neck. Keep your chin level.
• 🔤 Make the screen easier to read. Bigger text means less leaning in. Voice typing helps for long messages.
• 🪑 Sit well. Slumping on a sofa tips your head even further forward.

💻 How should you set up a laptop or desk screen?

A laptop is awkward by design. If the screen is at a good height, the keyboard is too high for your arms. If the keyboard is right, the screen is too low and your head drops forward.

• 🖥️ Raise the screen until the top of it is at about eye level, at arm's length. A stand, or a stack of sturdy books, works.
• ⌨️ Use a separate keyboard and mouse so your hands can stay low while the screen stays high.
• 💪 Keep your elbows by your sides, bent at a right angle, with your forearms resting lightly on the desk.
• 🪑 Sit back in your chair and keep your ears roughly above your shoulders.
• 🛋️ Avoid the sofa or bed for long sessions. If you must, prop the laptop on pillows so you don't look down.

⏱️ What is a daily reset for your neck and upper back?

Every 30 minutes, do a one-minute reset:

1. Sit or stand tall and look straight ahead.
2. Chin tuck. Glide your chin straight back, as if making a gentle double chin. Hold for 5 seconds. Repeat 5 times.
3. Shoulder blade squeeze. Draw your shoulders back and down. Squeeze your shoulder blades together for 5 seconds. Repeat 5 times.
4. Roll your shoulders and walk a few steps.

Once a day, if your neck and upper back feel stiff after long screen hours, add these two moves. They take about 4 minutes.

Shoulder-top ball release (2 minutes each side). Place a firm small ball between a wall and the muscle between the base of your neck and your shoulder blade. Lean in until the pressure is firm but not sharp. Slowly swing that arm across your body, up overhead and then behind your back. Tilt your head gently away as you go.

Upper back roller stretch (2 minutes). Lie on your back with your knees bent and a foam roller across your mid to upper back. Fold your arms across your chest. Breathe in. As you breathe out, let your upper back gently arch over the roller. Shift the roller a few centimetres after each breath, covering 3 to 5 spots. Keep it off your lower back. If your neck strains, support your head with your hands.

🩺 When should you see a doctor?

Most screen-related neck aches settle within a few weeks of changing habits. See a doctor if you have:

• 💥 Pain, tingling, numbness or weakness running into an arm or hand. This can be a pinched nerve in the neck.
• 🖐️ Clumsy hands, trouble with buttons or handwriting, or unsteady walking.
• 🌙 Neck pain that doesn't ease after several weeks, or wakes you at night.

Wear and tear in the neck is common with age, and usually harmless. See Cervical Spondylosis (Neck Arthritis). A check-up can tell what is behind your pain.
